Effective Date
All effective dates will be on the first (1st) of the month. Any enrollment application received by the 25th of the month will be effective on the first of the next month. All enrollment applications received after the 25th will be effective on the first of the following month. Example: Date received – Jan. 26th / Effective date – March 1st.

Changes in Plan / Cancellation Policy
For your protection we require that you notify American Health Choice in writing of any status change you wish to make in your plan. AHC will accept cancellations in writing only. You may cancel your plan at any time, however you will not receive a refund for any month in which your plan was in force/active. Change/Cancel Form

These statements refer to the Discount Portion of the Plan Only

The discount medical card program is NOT health insurance.
The plan provides discounts at certain health care providers for medical services.
The plan does not make payments directly to the providers of medical services.
The range of discounts for medical or ancillary services provided under the plan will vary depending on the type of provider and medical or ancillary services received.
The plan member is obligated to pay for all health care services but will receive a discount from those health care providers who have contracted with VantageAmerica Solutions, Inc., a discount medical plan organization.

ATTENTION MARYLAND RESIDENTS: Some discounts under the Physician and Hospital Referral Plan benefit are not applicable in Maryland. Discounts are not available for all In-Patient Procedures and certain Out-Patient Procedures under Maryland law. Out-Patient Procedures at network hospitals such as laboratory and diagnostics services are eligible for the discount.

NOTE: This discounts plan is not a qualified health plan under the Affordable Care Act (ACA). Spouse includes those legally recognized as having spousal rights under Civil Unions and Domestic Partnerships.

This plan offers a money back guarantee if canceled within 30 days. (Registration fee is non-refundable unless prohibited by law.
